Guidelines for Group Membership
- Group is defined as a body of health ministers/FCN’s from an agency, institution, congregation or incorporated health ministry network.
- Memberships would need to be coordinated, batched and mailed together, with one check from the agency, institution, congregation or incorporated health ministry network.
- Each person from the group must complete a membership form (see website to download the form)
- All membership forms will be batched and sent to HMA Office (see address below) along with one check.
- HMA will process the group memberships and send individual confirmation emails to each member.
- Groups will strongly encourage members to be actively involved by volunteering time on HMA committees and work groups.
NOTE: Cannot duplicate special offers and discounts (i.e. if your group is between 10 – 20 and you have individuals who are 65 and older, the group will still get the discount of $85/person, those individuals would need to join individually to receive the senior discount of $70).
