As the largest alliance of its kind, C-TAC (Coalition to Transform Advance Care) is bringing the best and brightest together to identify and implement evidence-based practices to improve advanced illness care. Please join us on March 2-3, 2015 for the National Summit on Advanced Illness Care, at the Institute of Medicine, National Academy of Sciences in Washington, DC.

Community and faith leaders will be represented at the Summit — ensuring that CTAC and the work of their members are grounded in community perspectives. C-TAC is advancing the movement for high quality, cost effective, patient-centered care for the advanced illness and end of life population. Speakers include Rev.
